Yes! I have maintained all my contracts that I have previously held with insurers. If I have accepted your insurance in the past (and your insurance hasn’t changed) then that will continue to be the case.

It depends. If you have a deductible that you don’t anticipate meeting over the entire year, then you may want to opt to be a cash/private pay patient. If you do not opt out of insurance billing then yes, you would be responsible for both the membership fee and any appointments prior to your deductible being met.

Yes and no. If you anticipate only seeing me once a year then you may want to choose the “2 payments of $185” option, as the the second payment of $185 is only collected if you have 2 or more visits.

Not yet. One of the reasons I have instituted this model is to have a more accurate sense of how many active patients I have so I can ensure follow-up visits and portal communication in a reasonable time period. If/when my patient panel is full I will stop accepting new patients and anyone interested in establishing or re-establishing care with me can be put on a waitlist and contacted when there is an opening.

Select “Cash/Private Pay”, with the understanding we can amend your selection if you insurance does end up covering your visit. For any insurance that I am not 100% confident about insurance coverage, I will charge you my cash/private pay rates at the time of your first visit AND I will also submit to your insurance. If your insurance does cover then your previous payment for your visit will be applied to the membership fee and you will resign the membership agreement selecting “New Patient Insurance Pay”. If your insurance does not cover, then you will remain a cash/private pay patient.

As long as my patient panel is not full you can schedule an appointment with me at any point over 2025. The membership fees are unchanged regardless of when you first appointment takes place in 2025.

No, the return membership fee is the same price, regardless of when your fist appointment takes place over the year. New patients (establishing care for the first time) do have a prorated membership fee depending on the date of their first appointment.

No. Every year you will choose whether or not you want to continue your membership.

No, the membership is not refundable. If you are a new patient and choose not to continue care after your first visit you will not be charged the second payment of the membership fee.

No, discontinuing care during the membership period will not result in a refund of membership fee. You can choose to discontinue your care at anytime and your membership will naturally expire on December 31st.
